Clearview provides individualized medical care including an onsite physician, consultants, 24-hour skilled nursing services, and intensive therapy services. Clearview specializes in caring for individuals with complex nursing needs including IV medications, tube feeding, and wound care.
Clearview Social Services are here to assist with all your discharge planning needs:
- Facilitate Treatment Team conferences to review progress and develop an appropriate discharge plan as desired.
- Assist with identifying discharge needs and services and make the appropriate referrals to maximize level of independence at home.
- Coordinate a home assessment with therapy staff to determine that home is safe and accessible.
- Help with the adjustment to the rehabilitation setting and assist as needed with maximizing experience while in short term rehabilitation.
